Key Review Excerpts

The staff at Greenbriar are exceptional or at least that was our experience. Amanda (admissions), Anthony (family services), and especially Bailey and her nursing colleagues were always available, provided excellent support, and demonstrated genuine compassion and care.

Long-term resident's family · 2025★★★★★

The facility lacks a 'rehabilitative unit' and the rehabilitative services as advertised. After a loved one was placed in this facility post surgery, she went multiple days after arrival with zero PT activity or even PT care planning.

Rehab patient's family · 2023☆☆☆☆

My friend is currently a patient, the food is horrendous, one day for dinner she recieved a cup of ham salad, 2 milks, and 2 coffee. Balanced? She became ill, sent to ER, retuned to the facility, 3DAYS later she still had vomit in her hair and on her body!!

This facility has an ongoing pattern of deficiencies across multiple surveys, with families filing complaints about safety hazards, medication errors, and communication failures. The most recurring problems involve medication management, care planning, and safety oversight. While all cited issues show correction dates, the persistence of deficiencies across different time periods suggests systemic challenges that families should carefully evaluate before choosing this facility.
