During observation while on tour on January 27, 2026, the kitchen appliances were observed on casters (wheels). This appliance did not appear to have a restraint chain or any other restraint apparatus that would prevent strain breakage of the gas connection. The Director of Maintenance confirmed there was no restraint chain or any other restraint apparatus on the appliances
Based on observation and document review, the facility failed to ensure the patient corridors were in good working condition. Failing to maintain doors could cause harm to patients and/or staff during an emergency
