Mona Assisted Living Home

Based on record review and interview, the manager failed to ensure a resident had a service plan that was reviewed and updated at least once every three months for a resident receiving directed care services, for one of one resident sampled, who received directed care services. Findings include: 1. A review of R1’s medical record contained a service plan dated March 29, 2025, for the directed level of care. There was no current service plan available for review. 2. In an interview, E1 reviewed R1’s medical record and acknowledged there was no current service plan available for review during the survey.
