What this means for your family

While Fountainview has a strong rehabilitation team that has helped many residents recover, the recent increase in reports regarding neglect and facility maintenance is concerning. We strongly advise families to tour the facility, specifically checking the cleanliness of the grounds and patient rooms, and to ask management directly about their current staffing ratios and incident follow-up procedures.

Key Review Excerpts

My wife was in Fairview Rehabilitation for more than 3 months. I have no words to express our thanks and appreciation, to the nurses and their staff, they were very attentive and helpful to her care.

Rehab patient's spouse · 2024★★★★★

My family member has been at Fountainview since February 2025. Though some of the staff is helpful/friendly, the medical care has been neglectful. He has not received oxygen as needed, there is cigarette odor present in the room, the room has been infested with ants several times.

Long-term resident's family · 2025☆☆☆☆

If you want to get a real feel for this place, park in the spots behind the building. You will see garbage strewn about everywhere - it is absolutely disgusting and they should be embarrassed.

Fountainview Care Center has significant concerns with multiple families filing complaints that triggered federal investigations revealing serious issues with resident protection from abuse and neglect, medication management, and resident rights violations. The facility shows a troubling pattern with abuse prevention deficiencies appearing repeatedly, including failures to properly respond to alleged violations and protect residents from restraints. While most issues have correction dates, the recurring nature of complaints and recent violations in 2025 suggest ongoing quality concerns families should carefully evaluate.
