In Room 5 there was an unsecured oxygen tank.
Facility failed to provide documentation of fire drills within the last 12 months for the 1st quarter day and noc shifts.
In Room 16 there were two 3-unit unfused cube adapters in use.
The fire sprinkler riser room sign was faded and no longer legible.
Facility failed to provide 30-second monthly activation tests on the emergency lighting for February through May of 2024.
In the office breakroom, an outlet cover was missing over the electrical outlet.
In the storage room of the big dining room there was an open junction box with exposed wiring.
There was a penetration in the wall of the employee breakroom.
Loaded sprinkler heads found in kitchen storage room.
Two carbon monoxide detectors were missing, one each in both dining rooms.
Facility failed to provide the 90-minute test on the emergency lighting within the past 12 months.
