FACILITY EVALUATION REPORT

The facility was visited for a case management follow-up regarding a resident elopement. The investigation confirmed that the facility failed to supervise the resident, who left the facility unassisted. This constitutes a Type A deficiency, resulting in a citation and a $500 civil penalty.

COMPLAINT INVESTIGATION REPORT

This report details a complaint investigation conducted on 07/03/2025 regarding allegations of staff causing injuries and failing to treat residents with dignity. The investigation found that the allegations were unsubstantiated based on resident and staff interviews, and record reviews.

The facility underwent a required annual inspection on March 14, 2025. The physical plant, food service, and documentation were generally reviewed, with positive observations noted in several areas. However, one Type A deficiency was cited regarding unsecured cleaning supplies on the second-floor balcony.

This was an unannounced case management follow-up visit to review a Plan of Correction (POC) due on May 22, 2024. The primary deficiency noted relates to bathroom sinks that do not allow residents to adjust water temperature. The facility is currently negotiating with contractors to address this issue.

The facility underwent an Annual Required Evaluation on May 15, 2024. The inspection noted one deficiency related to the maintenance of plumbing fixtures in the resident bathrooms. This deficiency requires the facility to ensure all necessary facilities are operational and properly equipped.
