The facility failed to ensure that a resident had a physician's order to self-administer medications. Specifically, Resident #4 was observed with an open bottle of vitamin B12, but there was no physician order, assessment, or care plan documentation authorizing the self-administration of vitamins.

The facility failed to ensure health care referral and follow-up for a resident by not notifying the primary care provider of high blood pressure readings. Specifically, blood pressure readings exceeding the physician-ordered threshold of 170 were documented in June 2022 without evidence of provider notification.

The facility failed to ensure that one of three sampled residents was properly tested for tuberculosis disease upon admission. Specifically, for Resident #3, while a second TB skin test was administered on the date of admission, there was no documented read date or result for that test.
