The facility failed to maintain a matching therapeutic diet menu for all physician-ordered therapeutic diets to guide food service staff. Specifically, for 4 out of 2 sampled residents, there was no therapeutic menu available for a no-concentrated sweets (NCS) diet. Staff were instead relying on a diet type report and personal judgment to determine appropriate meal components.
