Key Review Excerpts

The activities department sets it self apart from all the other facilities that we looked at for my mother. She is kept busy with lots of events and there are 2 separate activities rooms and a dining room.

Long-term resident's family · 2024★★★★

The absolute lack of staff, having 33 residents in one wing to 2 LPN'S and one RN who won't help or don't anything is an absolutely travesty. I can't say I did anything to help, but I couldn't sit here with all the residents literally calling out for help while their help lights were on with no one to come.

Visitor · 2024☆☆☆☆

My husband was there for rehab. They did not give him the proper food for his diet, even after meeting with the DON and kitchen manager. The dead cricket was in the room very visible for at least a week, even after showing housekeeping.

Six families filed complaints that triggered inspections, revealing serious concerns including inadequate notification to families about resident injuries or health changes, deficient care planning, unsafe respiratory care, and nursing staffing violations. This facility shows recurring problems with fire safety systems, medication management, and infection control that persist across multiple surveys.
