Key Findings
The most recent July 2025 inspection identified several issues regarding staff background check documentation, employee reference checks, kitchen sanitation, and fire safety hazards related to electrical use. Previous inspections in 2023 and 2021 showed the facility was in substantial compliance.
Jul 25, 2025Routine
Multiple direct care staff did not verification reference checks had been completed, as required.
Bathrooms #1 and #4, which did not have windows, also did not have forced ventilation to the outside, as required.
Prospective Administrator, Ashley Coselman, who was also working in the capacity as a direct care staff, did not have an eligibility letter available for review documenting she had fingerprints completed through the Workforce Background Check system.
The garbage container in the kitchen did not have a tight fitting lid available.
A dryer in the laundry room was plugged into a power strip rather than an outlet, therefore, creating a fire hazard. Additionally, dryer lint was observed on the power strip in the various power strip outlets also creating a fire hazard.
