What this means for your family

This facility offers a highly compassionate nursing staff and an excellent activities program that can greatly improve quality of life for long-term residents. However, families must be extremely vigilant regarding medical oversight and facility cleanliness, as there are documented patterns of delayed medications and poor room maintenance. We recommend conducting unannounced visits to check on hygiene and response times.

Key Review Excerpts

Upon arrival to Country Oaks we were greeted by literally everyone in the facility and immediately felt safe. When it came to my Mom it was all hands on deck, no exaggeration.

Long-term resident's family · 2025★★★★★

Took hours to get him his pain meds for his severe pain. He had roaches in his room. His trach was supposed to be cleaned as close to every 2 hours as possible due to the infection and it was not done once out of the 16 hours he was there.

Long-term resident's family · 2022☆☆☆☆

The staff is so nice and hospitable. They tend to your every need. As a patient i consider them family in a sense.

Country Oaks Care Center has a concerning pattern of 106 deficiencies across 24 surveys, with families filing 38 complaints triggering federal inspections. The facility repeatedly struggles with care planning, infection control, and nursing staffing adequacy, with the same issues recurring across multiple surveys despite reported corrections. Recent violations in 2024-2025 involving accident prevention, resident abuse protection, and basic care standards suggest ongoing systemic problems that warrant careful consideration before placement.
