Questions for Your Tour

  • 1Given the high praise for your therapy team, how do you ensure that same level of dedicated attention carries over into daily nursing care and hygiene assistance?
  • 2I noticed that response times to call buttons are a common concern; what specific protocols or staffing adjustments do you have in place to ensure residents are attended to promptly, especially during night and weekend shifts?
  • 3How does your nursing team coordinate with the pharmacy and medical staff to prevent medication errors and ensure that prescribed treatments are administered accurately?
  • 4Since dietary management is a priority for my family member, could you walk me through how you track and verify that residents on special diets receive the correct meals at every sitting?
  • 5I see that you actively respond to feedback online; how do you use that input to improve the quality of care and communication with families?
  • 6With your current staffing levels, how do you ensure that residents receive consistent support with daily hygiene needs and personal care throughout the entire day?

Families have filed multiple complaint reports triggering nearly half of the deficiencies found at this facility. Recurring issues include resident assessment and care planning, safety supervision and accident prevention, and infection control problems that have persisted across multiple surveys from 2023 to 2025. While the facility has corrected each violation after citation, the pattern of repeated problems in core care areas suggests ongoing quality concerns families should carefully consider.
