What this means for your family

While the facility has a history of strong rehab services, the recent transition to private ownership has led to consistent reports of understaffing and communication gaps. If you are considering this facility, we strongly advise you to visit during off-hours and weekends to observe staffing levels firsthand and ask management specifically how they address the reported shortages of basic supplies.

Continental Nursing & Rehab Center (formerly Wesley Place) has experienced a significant decline in reputation following its transition to private ownership under Aperion Care. While some families still praise the therapy team and individual staff members, many recent reviews highlight critical issues with understaffing, poor communication, and a lack of basic supplies. Potential families should be aware of the stark contrast between long-term positive experiences and recent reports of neglect and operational instability.

Key Review Excerpts

My mom had stayed here twice before for rehab with the stay being decently satisfactory. She is presently here for her third time, but now Aperion Care owns or manages the facility... Now, basic, essential supplies like towels and gowns are in short supply; staffing is significantly reduced.

Long-term resident's family · 2024★★★☆☆

My rating should be minus zero. The lack of competence, care, and most of all COMMUNICATION!!!!! is way below professional. Half the staff don't know what they're doing, and some of the others just don't care.

Family member · 2026☆☆☆☆

My loved one was just admitted. I thought I did a good job researching and chose this place because of the high staff rating. But I just read a review about this place being purchased and since then the care and quality has diminished. I believe it.
