Recent inspections have identified several safety and care concerns, including blocked fire exits, improper storage of cleaning supplies, and unmonitored medical/assistive devices on beds. There have also been issues regarding proper food labeling and maintaining up-to-date tuberculosis screening records for new residents.

The fire exit had a chair, a dresser and other items which can prevent or delay residents, visitors and staff from exiting quickly in an event of an emergency.
A mop bucket, not being used, was observed in the kitchen with dirty water in it.
The unlocked kitchenette sink in memory care contained several containers of disinfected spray stored underneath.
Food was observed in the kitchen refrigerator without an open date (e.g., mustard, taco sauce, cocktail sauce, salad dressing).
Three residents’ files did not contain evidence of TB being administered within 12 months before admission.

Two residents had bed rail assist bars that had a gap large enough for a limb to slip through (entrapment zone) and the devices were loose and shook easily. Additionally, there was no physician's order, service plan information, or specific staff training documented for these devices.
