The facility failed to maintain floors in good repair, specifically in the kitchen, dining room, and the threshold between the two areas. Observations revealed torn, peeling, and raised vinyl floor covering, including a 5-inch tear in the kitchen and sections exposing bare sub-flooring in the dining area. The Administrator acknowledged awareness of these issues since a January 2024 inspection but noted that repairs had not yet been completed by the building owner.
