The most recent inspection in September 2025 identified several concerns regarding resident privacy, medication documentation, food storage, and the unsecured storage of cleaning supplies and medication carts. While previous reports showed substantial compliance, the facility was required to submit a corrective action plan to address these safety and privacy risks.

Staff person 1 (SP1) employee record revealed the employee received tuberculosis (TB) testing prior to the start date of employment (noted in context of screening requirements).
Hazardous materials (bathroom cleaner) were left unlocked and accessible to cognitively impaired residents; medication treatment carts were also found unlocked.
Multiple unattended computers were left open displaying resident names, compromising resident privacy.
Medication Administration Records (MAR) showed staff failed to initial medication administration for Resident A (multiple dates), Resident B (multiple dates), and similar findings for Residents C and D.
Walk-in freezer contained various food items that were not appropriately covered or dated.
