This report details the findings of a complaint investigation regarding allegations of rough handling causing a fracture. The investigation reviewed records, interviewed staff, and analyzed video footage. Ultimately, the allegation was determined to be unsubstantiated due to a lack of evidence.

The facility underwent an unannounced Case Management visit on August 29, 2025. The Licensing Program Analyst observed the facility operations, including food storage and dining areas, and interviewed the Administrator. No deficiencies were cited during the inspection.

The facility underwent a required annual inspection on May 19, 2025. The inspector noted that the facility was well-prepared, with adequate food supplies, proper temperature controls, and fully operational safety equipment. No deficiencies were cited during the inspection.

The facility underwent a Required Annual Inspection on May 24, 2024. The inspection covered the physical plant, records, and interviews with staff and residents. No citations were issued during this inspection.

The facility underwent a Required - 1 Year Inspection on June 20, 2023. The inspection included interviews with residents and staff, and a review of resident and staff files. Two Technical Violations were issued, which relate to regulations that do not pose an immediate risk to health or safety.

The inspection was conducted unannounced to verify antigen testing compliance following a request from Contra Costa Health Services. The administrator confirmed that COVID-19 antigen testing had been completed for all staff present during the time an infected employee was present. No citations were issued during this visit.

The facility underwent an infection control annual inspection on 07/29/22. The inspector noted that the facility has an infection control plan in place and that various safety measures, such as daily cleaning, proper storage of chemicals, and functional detectors, were observed. No deficiencies were cited during this visit.

The facility underwent an unannounced Infection Control Inspection on 06/16/2021. The inspector observed positive measures, including proper visitor screening, available PPE, and fully stocked handwashing stations throughout the facility. No deficiencies were cited during this inspection.
