Key Review Excerpts

My stay at Brookhaven Rehabilitation Center was truly enjoyable thanks to the recreation director and their incredible team. They put their heart and soul into everything they do.

This facility has a pattern of serious concerns, with families filing complaints about abuse prevention, nursing staffing, and resident protection. Recent inspections found deficiencies in abuse reporting, nursing staffing levels, care planning, and infection control, with some issues recurring across multiple surveys.
