The facility failed to ensure that residents admitted to the Special Care Unit (SCU) had documented signed SCU disclosure statements. Specifically, a review of three sampled residents revealed that the required disclosure documentation was missing from their records.

The facility failed to ensure that 3 of 3 sampled residents in the Special Care Unit (SCU) had documentation of the required SCU disclosure. Interviews and record reviews revealed that the facility did not provide or document the necessary disclosure information to family members upon admission to the unit.

The facility failed to ensure that 2 of 3 sampled staff members were competency validated via return demonstration for Licensed Health Professional Support (LHPS) tasks. Specifically, there was no documentation that a medication aide had completed validation for tasks including insulin administration, feeding techniques, and resident transfers.

The facility failed to ensure medication orders were clarified with the resident's physician for one of three sampled residents. Specifically, for Resident #2, there were conflicting orders for Oxycodone dosage and frequency between the physician's order, the medication review report, and the pharmacy's records, leading to incorrect administration of the medication.
