What this means for your family

This facility has a highly regarded physical therapy and activities department, which can be a significant benefit for recovery. However, given the recurring reports of slow response times and hygiene issues, families should conduct an unannounced visit and specifically ask about the nurse-to-patient ratio on weekends and evenings.

Key Review Excerpts

The Physical Therapy & Occupational Therapy staff Kristen, Johnathan, Kathleen and Rick command applause and should be recognized for the professionalism, skills, empathy and first rate care that they provided for me.

Rehab patient · 2023★★★★

My Mom had a recent stay at Blueberry Hill. Having read good and bad reviews, I was understandably a bit concerned. Thankfully there were no issues. I was impressed with how the staff worked with my Mom and myself to insure her comfort and safety.

Long-term resident's family · 2023★★★★★

First of all the overwhelming smell of urine as you enter the facility is nauseating. The response to request are either ignored or take weeks to resolve. From nursing to management to medical staff nobody wants to take initiative to get request addressed and all blame one another.

This facility has concerning recurring issues, with families filing multiple complaints that triggered inspections. The most frequent problems involve medication management, resident rights violations (particularly family notification and care decisions), and care planning deficiencies. While all documented issues show correction dates, the pattern of repeated deficiencies across nursing staffing, infection control, and safety systems suggests ongoing operational challenges that families should carefully evaluate before considering placement.
