The most recent renewal inspection from September 2023 identified several administrative and health-related deficiencies, primarily involving missing tuberculosis screening documentation for residents and staff, as well as a lack of on-site supervision during the third shift. The initial licensing report from February 2020 showed the facility was in substantial compliance.
