The facility failed to maintain an uncluttered environment free from hazards due to unsecured oxygen cylinders. Specifically, multiple oxygen cylinders were found sitting upright on the floor without proper racks, crates, or signage on resident room doors. Additionally, the facility lacked a designated storage area for these cylinders.

The facility failed to maintain hot water temperatures between 100 and 116 degrees Fahrenheit at four sampled resident fixtures. Observations showed temperatures as high as 126 degrees Fahrenheit in the common bathroom and handicap accessible sink. Staff reported that a local official had previously advised the facility to monitor these temperatures due to the heat risk.
