What this means for your family

While recent reviews suggest improvements in facility cleanliness and atmosphere, the history of severe neglect, including bedsores and hygiene failures, is a major red flag. If you choose this facility, you must closely monitor daily hygiene, skin integrity, and ensure that staff are responding promptly to call lights.

How They Respond to Reviews

45%response rate

The owner relies heavily on repetitive, templated scripts for positive reviews, often using the exact same phrasing across different users. While they do provide a more structured, professional response to negative reviews by directing users to a compliance line, the positive engagement lacks genuine personalization.

Key Review Excerpts

Palestine HC does not drug its residents in attempt to manage dementia residents' movement/ramblings. These nurses know how to work with and re direct dementia patients in a proper manner with respect and dignity.

Memory care family member · 2021★★★★★

They're always short staffed, and they take forever to answer a call. My boyfriend was sent here because they were the only facility within a 20 mile radius that would accept him. He suffered a stroke and was there to get better and for therapy.

Rehab patient's partner · 2023☆☆☆☆

This is the worst location in the world my mother went in just to learn how to walk again and she came out with the bed sores because you can’t stick your whole hand in here bottom because they didn’t toss her they didn’t turn her they didn’t clean her

18 complaint-triggered
$76,268 in fines

This facility has a concerning pattern of complaint-triggered deficiencies (17 out of 38), with families filing multiple reports about serious issues including abuse protection, infection control, and medication management. The most recurring problems involve preventing abuse and neglect, infection prevention, and fire safety systems. While most deficiencies show correction dates, the repeated nature of core safety issues—particularly abuse protection violations appearing across multiple complaint investigations—suggests ongoing systemic challenges that families should carefully consider.
