Specifically, the resident required wound care every other day until a home health agency could assume duties, but the necessary follow-up and referrals were not completed.

The facility failed to ensure that exit doors used by residents known to be disoriented or wanderers were equipped with functioning sounding devices. Specifically, alarms did not activate when the front entrance, laundry/pantry, and lounge area doors were opened. This failure occurred for residents identified as being intermittently or constantly disoriented.
