Evidence: Progress notes indicated that Resident #1 eloped from the facility?s special care unit on 10/4/25 at approximately 6:25 PM. Resident #1 proceeded to exit the building and ride the community shuttle. Facility documentation indicated that Resident #1 wanted to go home, but she did not know where to go. The shuttle driver informed facility security, and Resident #1 was escorted back to the unit. Resident #1 reportedly rode the shuttle bus for approximately 20 minutes. Resident #1's record contained an assessment of serious cognitive impairment form, dated 6/9/25, that states that she has a serious cognitive impairment and that she is unable to recognize danger or protect her own safety and welfare.

Evidence: A facility incident report indicated that Resident #2 was administered seven doses of Lyrica 150mg from 9/27/25 until 9/30/25. Resident #2's record contains a physical examination form, dated 2/14/24, that states that she is unable to self-administer her medications. Resident #2's record also included an order, dated 9/11/25, that calls for her to receive 50mg of Lyrica twice per day. Resident #2's progress notes indicated that she had a prescription refill from July 2025 for Lyrica 150mg, but no order for the increase was on file.

Evidence: Resident #1's record contained an Assessment of Serious Cognitive Impairment form, dated 7/28/23, that states that Resident #1 has a serious cognitive impairment with an inability to recognize danger or protect his own safety and welfare. On 10/15/24, Resident #1 eloped from staff supervision during an outing at a restaurant. Resident #1 was discovered to be missing during a head count performed before the group departed from the restaurant. Resident #1 was unable to be located by the staff attending the outing, and law enforcement was contacted. Resident #1 was located approximately 47 minutes after he was discovered missing by facility staff that searched the area. Resident #1 was located, without any identified injuries, in a nearby parking lot.
