What this means for your family

While the therapy department is highly regarded for helping patients regain mobility, families should be aware of significant, recurring reports regarding slow response times and hygiene issues. We strongly recommend visiting unannounced during off-hours and weekends to observe the actual level of care and staffing before making a decision.

Key Review Excerpts

The therapy at this facility is incredible and the food is great! Everyone was so welcoming and friendly!

Rehab patient · 2017★★★★★

My friend would need help setting up so she could eat or breathe easier or getting a bedpan and it took forever for a nurse to get in there to help her. She ended up having an accident in the bed and it took an hour for them to get her all cleaned up.

Friend of resident · 2022☆☆☆☆

I physically had to run numerous roaches away from crawling on the patients as well as in their beds. I witnessed a patient BEG for help to be gotten up out of bed... A nurse shunned her.

This facility shows concerning patterns with repeated deficiencies across multiple surveys, particularly in fire safety systems, emergency preparedness, and electrical maintenance. The most recurring issues involve sprinkler system maintenance, electrical safety testing, and nurse staffing information posting. While all deficiencies show correction dates, several problems persist across surveys, including sprinkler maintenance cited three times and electrical issues appearing repeatedly, suggesting ongoing challenges with building safety compliance that families should discuss during visits.
