Key Review Excerpts

The staff is incredibly dedicated and go above and beyond in providing personal care to the residents — both medically and physiologically. I can never thank each of them enough for the many, many incidents of behind-the-scenes care that they’d never know were noticed and appreciated.

Long-term resident's family · 2024★★★★

My dad was sent here for rehab, I arrived prior to him and the room they assigned was not ready, there was plastic and duct tape on the windows, they furniture was broken, the bathroom was missing tiles and the sink was covered in rust.

Rehab patient's family · 2026☆☆☆☆

The staff is loving and caring to both patient and family members. They also have a lot of help so your loved one doesn’t have to wait 45 to use the bathroom. The CNA’ are thoughtful, loving and kind as well as the nurses.

Families have filed numerous complaints triggering deficiencies, with recent issues involving failure to properly investigate and report suspected abuse, neglect, and theft of residents' belongings. The facility shows persistent problems across accident prevention, daily care assistance, and medication management, with many issues recurring across multiple surveys. While the facility reports corrections for most violations, the pattern of repeated complaint-driven investigations and similar deficiencies suggests ongoing care quality concerns that families should thoroughly investigate during visits.
