Key Review Excerpts

The smell of urine and feces and food is always mingled together and strong. Pills in the floor, spills on the wall, gross.

Family member · 2025☆☆☆☆

I’ve sat and rang the nurses bell and waited about 44 min for one to come to the room, with my mom having seizures and all you would think that it wouldn't take that long.

Family member · 2024☆☆☆☆

The wound care nurse, I believe her name was Tanya’ or Toya. She made a difference, she mad us feel warm and comfortable. Tanya got us a remote control and instantly had housekeeping clean my dad’s bathroom.

Families have repeatedly filed complaints triggering most deficiencies, raising serious concerns about resident protection from abuse and neglect, accident prevention, and treatment quality. These core care problems—particularly inadequate supervision and safety measures—persist across multiple surveys despite corrections. While the facility reports fixing all cited issues, the recurring pattern of complaint-driven investigations suggests ongoing quality problems that families should carefully consider before placement.
