A special investigation conducted in December 2023 identified a serious incident where a resident sustained an unexplained skin tear and bruising. The investigation also revealed that the facility failed to properly document the injury or the cause of the injury in the resident's medical records, contrary to facility policy.

Resident A was not treated with dignity including protection and safety; specifically, the resident was observed with a skin tear and bruising (appearing to be a large handprint) on his forearm of unknown origin, and there was no documentation in the resident's record or incident report regarding how or when the injury occurred.
Failure to follow facility policy and training regarding the documentation of incidents, falls, injuries, or changes in condition in the incident reports and the EMAR daily log.
