Key Review Excerpts

The staff that works back in memory care are phenomenal. They are a strong team that takes care of the patients and work together! Now the administration team that's up front starting with the DON(Mandy Johnson) she is unprofessional, not a team player, manipulative and FAKE!

Family member · 2026☆☆☆☆

My dad was a resident there and every day that I have visited his sheets were absolutely filthy. He was urinating through his brief and instead of changing him more often they were putting extra blankets down wich gave him sores on his behind.

Daughter of resident · 2026☆☆☆☆

Doesn't always have adequate staff and so doesn't answer patient calls. Refuses to give patient their test results to share with other care providers. Only checks blood sugar and pressure when patient requests, if they can get someone to answer the call.

53 complaint-triggered

Addison Heights has a concerning pattern of repeated complaint-triggered deficiencies, with nearly half (48 of 102) stemming from family reports. The facility shows persistent issues with maintaining a clean, safe environment, medication management, and infection control across multiple surveys. While deficiencies are reportedly corrected, the recurring nature—particularly cleanliness violations appearing in seven separate complaint investigations—suggests ongoing operational challenges that families should carefully evaluate before considering placement.
