Based on record reviews and interviews, the facility did not ensure that residents complete a risk assessment for tuberculosis annually, as evidenced by the completion of the current screening form published by the Virginia Department of Health or a form consistent with it. 1. The record for resident 4 did not contain documentation showing resident 4 had completed a risk assessment for tuberculosis within one year since the last risk assessment. The last risk assessment for tuberculosis for resident 4 was dated 12/26/2022 and then 01/09/2025. 2. The record for resident 5 did not contain documentation showing resident 5 had completed a risk assessment for tuberculosis within one year since the last risk assessment. The last risk assessment for tuberculosis for resident 5 was dated 11/06/2020 and then 07/10/2025. 3. Based on observations made during the medication cart audit, the facility failed to ensure that over-the-counter medications remain in their original containers and are labeled with the resident's name, or in a pharmacy-issued container, until administered. Evidence: 1. On September 19, 2025, at approximately 1:16 p.m., the Licensing Inspector (LI) conducted an audit of the medication cart. Three bottles of dietary supplements were observed without labeling or any other identifiable information on the containers. During the inspection, it was noted that the cart contained the following items: a. One bottle of Move Free dietary supplement. b. One bottle of Care One vitamin D3 125 mcg tablets. c. One bottle of Quicksilver Liquid Ultra vitamin supplements. 2. Staff 6 confirmed the LI's findings. 3. Photo evidence taken.

Based on licensing inspector (LI) observations and interviews, the facility failed to comply with the facility's own policies and procedures. Evidence: 1.On 11/8/2024, collateral 1 was observed by the LI and staff 1 in the second-floor conference room of the facility. Collateral 1 was observed with two prescription bottles of medications for resident 1. The prescription pills were observed on the table and collateral 1 was observed using a pill splitter to split the pills into halves. 2.
