Based on observation and interview, the manager failed to ensure that equipment and food contact services were clean, which posed a health and safety risk to residents. Findings include: 1. During an environmental inspection, the compliance officer observed the inside oven door bottom and shelves were soiled with a black and brown substance. 2. In an interview, E1, and E4 acknowledged the oven was not clean.

Based on observation, and interview, the manager failed to ensure a rechargeable fire extinguisher had a tag attached to the fire extinguisher that specified the dated of the last servicing and the identification of the person who serviced the fire extinguisher findings include: 1. During an environmental inspection, the Compliance Officer observed a rechargeable fire extinguisher in the kitchen area. The fire extinguisher did not have a tag attached to the fire extinguisher that specified the date of the last servicing and the identification of the person who serviced the fire extinguisher. 2. In an interview, E1 and O1 reported the fire extinguisher was inspected recently, and acknowledged the fire extinguisher did not have a tag.

Based on observation and interview, the manager failed to ensure a smoke detector, installed in a caregiver occupied bedroom, was in working order. The deficient practice posed a health and safety risk if a smoke detector was not working during an emergency. Findings include: 1. During an environmental inspection with E3 and E5, the compliance officer observed a stairway leading to a basement of the house where a bedroom was located and occupied by E5. The bedroom ceiling was observed to have wires hanging, where a smoke detector had been removed. 2. During an interview, E5 reported there had a been a problem with the smoke alarm system, and acknowledged the smoke alarm was absent from the ceiling. E1, O1, O2, and E5 acknowledged the bedroom is required to have a working smoke alarm.

Based on observation and interview, the manager failed to ensure a bathroom which was accessible from a common area contained paper towels in a dispenser or a mechanical hand dryer. The deficient practice posed a potential infection control risk. Findings include: 1. During an environmental inspection, the compliance officer observed a bathroom accessible from a common area had a paper towel dispenser; however, the dispenser did not have paper towels in it. Another bathroom located by resident bedrooms also had a paper towel dispenser with no paper towels. 2. In an interview, E4 acknowledged the bathrooms did not have paper towels.
