What this means for your family

This facility offers excellent rehabilitation therapy and a modern, clean environment that many families appreciate. However, the recurring reports of slow response times and medication management issues on weekends are significant. We strongly recommend visiting on a weekend to observe staffing levels yourself and asking for a direct contact plan for nursing management.

YAMATO NURSING AND REHABILITATION CENTER (often referred to as 'The Encore') receives a high volume of positive reviews praising its clean, modern facility and the effectiveness of its physical and occupational therapy teams. However, a significant number of critical reviews highlight serious concerns regarding understaffing, slow response times to call buttons, and poor administrative communication. Families should be aware that while many report excellent experiences, there is a recurring pattern of complaints regarding care consistency, particularly on weekends.

Key Review Excerpts

The physical and occupational therapy is amazing. When I arrived at the encore I was in a wheelchair and when I left I was able to walk with a walker.

Rehab patient · 2025★★★★★

The DON Brandy never answers her phone or email . You come in the building to try to talk to her in person she’s ALWAYS in a meeting . My husband was waiting hours to speak to her and yet she failed to come out the meeting to handle our concerns.

Family member · 2025☆☆☆☆

My dad has run out of his pain medicine 3 times over a weekend when everyone magically becomes unavailable and I had just spoken to the doctor about this issue.

This facility shows persistent issues across all three surveys, with recurring problems in food safety, medication management, and infection control that have not been fully resolved despite corrections being reported. Food handling and storage violations appeared in all three inspections (2022, 2023, 2024), medication labeling issues recurred twice, and infection control deficiencies persisted across surveys. While the facility corrects cited problems, the pattern suggests underlying systemic challenges that families should discuss during visits.
