What this means for your family

While Wisteria Gardens has a history of strong physical therapy outcomes, recent feedback indicates a concerning decline in the quality of care for dementia patients and general staff responsiveness. If you are considering this facility, we strongly advise asking for a tour of the specific unit your loved one will occupy and inquiring about their specific protocols for dementia safety and night-shift staffing ratios.

Questions for Your Tour

  • 1I noticed your administrative team is very active in responding to feedback online; how do you use that resident and family input to improve the daily experience here?
  • 2With your strong focus on physical and occupational therapy, how are these programs integrated into the daily routine for residents who need extra support?
  • 3Given the specific needs of residents with memory loss, what specialized safety protocols and engagement activities do you have in place to ensure they remain secure and stimulated?
  • 4I see your staffing rating is quite high; could you explain how you ensure that same level of attentiveness and professionalism across all shifts, particularly with bedside care?
  • 5What is your current process for facility maintenance and housekeeping to ensure that common areas and resident rooms remain clean and well-kept?
  • 6How do you handle medical emergencies or urgent health changes, and at what point is our family typically brought into the communication loop?

Key Review Excerpts

If you have a family member with dementia, DO NOT send them to Wisteria Gardens for rehab. They are not equipped to handle dementia patients, and the experience we had there with my father was terrible.

Memory care family member · 2024★★★☆☆

Aide's/care assistants there are rude, unsmiling, unconcerned, and do not want to be 'bothered' to be asked do their jobs of assisting residents with mobilization, self care, changing etc. Don't expect a call button to be answered in a timely manner if ever.

Long-term resident's family · 2024☆☆☆☆

The doctors and nurses/cna’s are so awesome! Physical therapy team top of the line and a shout out to Ms. Margie for taking care of business!

6deficiencies
Near state avg (6.7)

Wisteria Gardens has recurring issues with food safety and infection control that persist across multiple surveys, with food procurement violations appearing in both 2023 and 2025 inspections. The facility also shows repeated problems with medication management, resident care planning, and infection prevention. While all 16 deficiencies across three surveys have been corrected according to state records, the pattern of similar violations reappearing suggests ongoing operational challenges that families should discuss during visits.
