Key Review Excerpts

The place is clean, no odors. I have my mental alertness, and I’m pleased with care. The food is better than I thought it would be , nursing is very responsive. Therapists are committed, caring.

Rehab patient · 2024★★★★

Although it is one of the older facilities we toured, it was the cleanest and had the nicest staff. They were extremely attentive but the ones that were the absolute best that we worked with were both of the Crystals, Christine, and her CNAs Keisha and Bianca.

Memory care family member · 2026★★★★★

My 98 year old daddy fell twice in 4 days and both times had to be taken to the ER. First time I was not given a clear picture of what happened but the second fall could have been prevented.

White Oak Manor has concerning patterns, with nearly half of its 31 deficiencies stemming from family complaints about serious care issues. Recurring problems include protection from abuse and neglect, nursing staff competency, and care planning. While the facility has corrected reported violations, the repeated complaints about fundamental safety protections and staff qualifications suggest ongoing quality concerns that families should investigate thoroughly before considering placement.
