What this means for your family

The facility is frequently described as a loving and caring environment for seniors. However, due to a severe report of medical oversight regarding bed sores, families should perform rigorous due diligence and ask specifically about their skin integrity monitoring protocols.

Questions for Your Tour

  • 1We've heard such wonderful things about the loving and compassionate nature of your staff; how do you ensure that this level of personal care is maintained with such a small, intimate group of 12 residents?
  • 2Since we are looking for a very close-knit environment, how do you keep families updated on their loved one's daily well-being and any changes in their health?
  • 3What specific routines or specialized support do you have in place for residents who may be experiencing early signs of memory loss?
  • 4Could you walk us through what a typical day of social activities and engagement looks like for the residents here?
  • 5In the event of a medical emergency during the night, what is your protocol for contacting both medical professionals and the family?
  • 6We noticed the positive reputation of your Graham location; how do you bring that same level of excellence and care to the Brock Building?

Key Review Excerpts

My husband was a patient at Springview Memory Care until he lost the ability to stand or walk. As soon as a skilled nursing facility in the area had space, he was transferred there, that decision having been made by Springview 2 weeks before the transfer. The initial assessment at the SNF the day he arrived revealed 13 bed sores, 2 of which are going to require surgery.

My mother stayed at the Graham location & they were wonderful! Little concern about neighborhood but no problems at all. Would highly recommend Springview when your parents need extra care!

Nutrition and Food ServiceD 310

The facility failed to serve therapeutic diets as ordered by the physician. Specifically, three residents who required chopped meats were not served food in the correct consistency, as evidenced by a resident being served an unchopped salmon patty.

Special Care Unit StaffD 465

The facility failed to maintain adequate staffing levels in the Special Care Unit to meet minimum regulatory requirements. On 30 sampled shifts, there were insufficient staff members present to provide necessary personal care and supervision for a resident census of 11-12 residents.
