The facility failed to serve therapeutic diets as ordered by the physician. Specifically, three residents who required chopped meats were not served food in the correct consistency, as evidenced by a resident being served an unchopped salmon patty.

The facility failed to maintain adequate staffing levels in the Special Care Unit to meet minimum regulatory requirements. On 30 sampled shifts, there were insufficient staff members present to provide necessary personal care and supervision for a resident census of 11-12 residents.
