Key Review Excerpts

The staff was very friendly and you could feel a peaceful vibe, and it was clean. The staff very attentive, matter of fact the resident accidentally hit the call bell and let me tell you a caregiver was in his room asking what he needed so fast.

Visitor · 2025☆☆☆☆

They have no help, smell of urine is strong, don’t have enough bedding for beds if residents wet the bed . Residents lay is feces and urine soaked diapers. Some with special dietary requirements are not given food to meet their needs.

Visitor · 2024☆☆☆☆

I was transferred to there from Penn State St Joseph on January 1st. I needed occupational and physical therapy plus some skilled nursing care. I was able to go home on February 18th thanks to the therapy's I received and nursing care.

This facility shows persistent problems with food safety, fire safety, and medication management, with families filing multiple complaints. Recurring issues include improper food handling and storage, fire safety violations (sprinkler systems, exit signs, blocked corridors), and pharmacy service deficiencies. While most deficiencies have been corrected, the pattern of repeated violations across the same areas and five uncorrected deficiencies from the most recent survey suggest ongoing operational challenges that warrant careful consideration.
