3-year lookback · Medicare 2026
This facility shows persistent problems with food safety, fire safety, and medication management, with families filing multiple complaints. Recurring issues include improper food handling and storage, fire safety violations (sprinkler systems, exit signs, blocked corridors), and pharmacy service deficiencies. While most deficiencies have been corrected, the pattern of repeated violations across the same areas and five uncorrected deficiencies from the most recent survey suggest ongoing operational challenges that warrant careful consideration.
