An initial certification survey was completed on 7/1/25. Deficiencies were cited. Based on observation and interview, the facility (residence) failed to ensure all first aid supplies were available, affecting all members (residents) who would be served by this residence. Findings include:On 7/1/25 at approximately 11:30 a.m., a first aid kit was brought over by the administrator' s external consultant from the sister residence next door that was yet to be licensed. However, the first aid kit was missing triangular bandages with safety pins and a cardiopulmonary rescusitation (CPR) barrier device or mask.On 7/1/25 at 11:55 a.m., the administrator stated he was unaware of what all was required to be in the residence' s first aid kit and would have to order the missing items. Based on observations and interview, the facility (residence) failed to ensure the secure outdoor area was level, well maintained, and appropriately equipped for the population served, affecting all members (residents) who would be served by this residence. Findings include:1. ObservationsOn 7/1/25 at approximately 10:38 a.m., an environmental tour of the west outdoor courtyard revealed the following: Barbed wire that was within arms reach, lining the top of the fence. There was no area that provided protection from weather elements. On 7/1/25 between 10:00 a.m and 1:00 p.m., an environmental tour of both the east and west courtyards revealed drop offs from the concrete sidewalk, measuring up to six inches. These drop offs posed a tripping hazard to residents who utilized the outdoor courtyards. 2. InterviewOn 7/1/25 at approximately 12:30 p.m., the administrator stated he was aware he was aware he was required to provide a level and well maintained secure outdoor area; however, he was unaware that the west courtyard needed a covered area to provide protection from weather elements.

Assisted living residences, as defined herein, shall comply with all applicable federal and state statutes and regulations including, but not limited to, the following: (A) this Chapter 7.Based on observation and interviews, the residence failed to be in compliance with all applicable regulations.This failure created the potential for mismanagement of the care and services for the residents who would be served by this residence. Findings include:-10.9 Each kit shall include, at a minimum, the following items: (A) Latex free disposable gloves, (B) Scissors,(C) Adhesive bandages, (D) Bandage tape, (E) Sterile gauze pads, (F) Flexible roller gauze, (G) Triangular bandages with safety pins, (H) A note pad with a pen or pencil, (I) A CPR barrier device or mask, and (J) Soap or waterless hand sanitizer.On 7/1/25 at approximately 11:30 a.m., a first aid kit was brought over by the administrator' s external consultant from the sister residence next door that was yet to be licensed.
