A relicensure survey with complaint #CO40277 was completed on 8/12/25. Deficiencies were cited. Based on observation and interview the residence failed to ensure that qualified medication administration persons (QMAP) are trained in and apply nationally recognized protocols for basic infection control and prevention when preparing and administering medications, affecting 9 current residents.Findings include:1. ObservationDuring medication administration on 8/11/25 from 7:55 a.m. to 8:05 a.m., Staff #5 failed to wash their hands prior to starting medication administration. Staff #5 administered medications to Resident #11, then Resident #8. After administering medications to Resident #8, Staff #5 blew their nose and failed to wash their hands after as well as prior to administering medications to Resident #6. Staff #5 failed to wash their hands after administering medication to Resid.. Based on observation and interview, the residence failed to have a locked cabinet to store medications when unattended by qualified medication administration persons (QMAP) or other licensed staff, affecting nine current residents. Findings include:1. Observation On 8/11/25 at 7:55 a.m., Staff #5 was administering medications to Resident #8 and failed to lock the medication cart prior to leaving the cart. The medication cart was unlocked and unattended in the living room of the residence with three other residents present. 2. InterviewOn 8/12/25 at 10:18 a.m., the administrator stated she would expect the medication cart to be locked at all times when unattended by staff. Based on observations, record review, and interview, the residence failed to comply with the Colorado Clean Indoor Air Act (CCIAA), affecting nine current residents.Findings include:1. ObservationsOn 8/11/25 at 8:13 a.m., Resident #2 was observed on the front outside patio smoking a cigarette in a chair about one foot from the brick wall of the residence and about 21 feet from the residence' s front door. Resident #2 was ashing their cigarette in a metal ashtray on a table and talking with the administrator. The residence had a designated smoking area that complied with the Colorado Clean Indoor Air Act on the front outside patio; however, the residence failed to ensure Resident #2 used it.2. InterviewOn 8/11/25 at 9:46 a.m., the administrator confirmed that Resident #2 smokes on the front outside pa.. She acknowledged needing to encourage Resident #2 to go to that area to smoke and to arrange the patio so she did not sit too close to the residence when smoking her cigarette. THIS PORTION OF THE REPORT IS FOR INFORMATIONAL PURPOSES ONLY.No response is necessary.The residence was advised it must review and maintain the following processes in accordance with existing program regulations found at 10 CCR 2505-10 8.7000.8.7550.C.1 Supportive Living Program services consist of structured services designed to provide: a. Assessment; b. Protective Oversight and supervision as defined at Section 8.7506.B.2;c. Behavioral Management and Education; d. Independent Living Skills Training in a group or individualized setting to support: i. Interpersonal and social skill development; ii. Improved household management skills; and iii.Other skills necessary to support maximum independence, such as financial management, household maintenance, recreational activities and outings, and other skills related to fostering independence. e. Community Participation; f. Transportation between therapeutic activities in the community; g. Activities of Daily Living (ADLs); h. Personal Care and Homemaker services; and i. Health Maintenance Activities. j. The Supportive Living Program provider shall ensure that each Member is furnished with their own personal hygiene and care items.
