The facility failed to maintain hot water temperatures within the required range of 100°F to 116°F. Observations revealed temperatures as high as 126.3°F in common area restrooms and as low as 74.0°F in the residents' common bathroom. Additionally, the facility failed to maintain proper temperature logs.
The facility failed to ensure all residents were tested for tuberculosis disease upon admission. Specifically, for Resident #3, there was no documentation of a TB test since their admission on 11/06/19.

Observations revealed medications stored in an unlocked cardboard box with stains at the bottom, which did not protect the medications from contamination or ensure proper identification.

The facility failed to ensure the number of residents did not exceed its licensed capacity of five. An observation revealed six residents living in the facility, including a sixth resident who had been moved in temporarily due to staffing issues at another location.
The facility failed to maintain walls, ceilings, and floors in good repair. Findings included black fuzzy spots on a bathroom ceiling, a large tear in the bathroom floor exposing sub-flooring, broken towel racks, and holes in the main hallway wall.
