The facility failed to administer medications in accordance with physician orders for 2 of 2 sampled residents. Specifically, for Resident #1, there was no documentation of Gabapentin administration for the 8:00am dose throughout December 2025, despite an active order for the medication.

The facility failed to ensure that evacuation capabilities for one resident were in accordance with the facility's licensed capacity. During a fire alarm test, Resident #2 did not respond to the alarm because they were asleep, despite being licensed as an ambulatory resident.

The facility failed to maintain the premises in an uncluttered, clean, and orderly manner free of hazards. Observations revealed holes in floorboards, walls, and ceilings, grime and dirt on surfaces throughout the facility, and the use of throw rugs in the hallway, living room, and bedroom.

The facility failed to keep floors in good repair, specifically noting a 4-foot area in the living room that sank 2 inches, a 2-foot area in resident bedroom #1 that sank 2 inches, and a hole in the hallway wood flooring. The Administrator was aware of the floor issues but had not yet completed the necessary repairs.

The facility failed to maintain floors and surfaces in good repair. Specifically, the wooden handrails on both the front and side entrance handicap ramps had multiple raised nails protruding from the top of the rails.
