The facility failed to administer medications in accordance with physician orders for 2 of 2 sampled residents. Specifically, for Resident #1, there was no documentation of Gabapentin administration for the 8:00am dose throughout December 2025, despite an active order for the medication.

The facility failed to ensure that evacuation capabilities for a resident were in accordance with the facility's current license. During a fire drill, one resident failed to respond to the fire alarm because they were asleep due to illness, which did not meet the required evacuation capability.

The facility failed to maintain the premises in an uncluttered, clean, and orderly manner free of hazards. Observations revealed holes in floorboards, walls, and ceilings, grime and dirt on surfaces throughout the facility, and the use of throw rugs in the hallway, living room, and bedroom.

The Administrator was aware of the floor issues but had not yet completed the necessary repairs.
