The facility failed to ensure that exit doors had working, audible alarms for a resident identified as constantly disoriented. Observations revealed that the alarms on both the front and back exit doors did not sound when the doors were opened.

The facility failed to ensure that a resident was not admitted solely for the treatment of a mental illness. A review of Resident #1's records showed a diagnosis of schizophrenia and medications used to treat schizophrenia and anxiety, without any other qualifying diagnoses listed to justify admission.

The facility failed to ensure that 2 of 3 sampled residents were not admitted for the treatment of a mental illness. Specifically, Resident #1 was admitted with a diagnosis of schizoaffective disorder and received outpatient psychiatric services, but the facility admitted the resident despite the prohibition on admitting individuals for the treatment of mental illness.
