This facility has 43 deficiencies across three years with recurring issues in care planning, medication management, and safety compliance that persist across all surveys. The most recent January 2025 inspection found problems with care planning development, medication oversight, and accident prevention. While all deficiencies show correction dates, the pattern of repeated violations in core care areas suggests ongoing operational challenges that families should discuss during visits.
