What this means for your family

While some visitors report positive experiences with the rehab and activities teams, the facility has a recurring pattern of serious allegations regarding resident hygiene, wound care, and safety. If you are considering this facility, we strongly recommend conducting an unannounced visit and specifically inspecting the resident's living area for cleanliness and asking about their protocols for repositioning and hygiene for high-needs residents.

Key Review Excerpts

My loved one was there only one week and developed a Stage 4 ulcer that lead to sepsis. We had to make Constant phone calls to the Director of Nursing Alexa/Alexis and to the nurses station to make sure that my loved one was getting meds, being turned, and receiving proper care.
