Employees; general provisions: Insufficient staff on the midnight shift to meet the needs of the residents. Review of service plans for Residents C and D indicated they required two-person assistance, but staffing levels were inadequate.
Resident medications: Resident A did not receive scheduled Hydromorphone multiple times during the midnight shift on 8/14/25 and 8/16/2025. Resident B had multiple missed doses or unchartered medications throughout August and September.

Admission and retention of residents: Resident A was admitted to the facility on 2/13/2022 but did not complete a T.B. screening until 2/14/2022. Additionally, the facility was unable to provide a T.B. risk assessment.
Employee's health: Review of employment files for Associates 1, 2, 3, and 4 revealed each employee did not receive a T.B. Screening prior to occupational exposure.
