FACILITY EVALUATION REPORT

The facility was toured, and observations confirmed that food supply, safety equipment, and general areas were compliant with regulations. The visit was a Case Management review where the facility was informed of an overdue license fee balance of $3,339.00. A Type B deficiency was cited regarding the failure to maintain current licensing fees. The facility must provide proof of payment or a payment plan by the specified date.
