What this means for your family
While the facility is physically attractive and some families report compassionate care, the recurring reports of neglect, poor hygiene, and communication failures are serious red flags. If you consider this facility, conduct an unannounced visit to the back floors and ask specifically about current staffing ratios and the recent turnover of management.

Key Review Excerpts
“My mother walked in to this facility in October 2020, and by December the first time I got to see her she was flat on her back in bed, wasn’t opening her eyes, could barely speak anymore. My mom fell 4 times in 3 months here out of her bed.”
“My father passed away yesterday but the care and respect they gave him is the reason I’m writing this review... When he passed he was draped in an American flag ( he was a vet) and the staff saluted him ans his body was leaving, some were crying.”
“The management staff was awesome until the past couple of months when literally the majority of upper management left within weeks of each other and a new team was brought in who are rude, incompetent, unprofessional and continually violate nursing home policies.”

Inspection History
Medicare Inspection History
3-year lookback · Medicare 2026
This facility shows concerning patterns with families filing multiple complaints that triggered inspections, primarily involving nursing staffing, care planning, safety hazards, and food safety. The facility has had persistent issues across key areas including nursing adequacy, accident prevention, medication management, and nutrition services that have recurred across multiple surveys from 2021 through 2024. While all deficiencies show correction dates, the repeated nature of problems in critical care areas warrants careful consideration before visiting.
