FACILITY EVALUATION REPORT

The inspection identified multiple deficiencies, including several Type A violations related to safety and resident rights, such as an unsecured emergency exit and improper placement of a bedridden resident. Additionally, Type B deficiencies were noted concerning the improper issuance of an eviction notice and providing incorrect guidance regarding resident medication management. The facility underwent an unannounced case management visit prompted by an incident report regarding a resident leaving the premises. The Licensing Program Analyst reviewed records and interviewed the administrator, concluding that the facility provided sufficient supervision based on the resident's self-responsible status. No immediate health or safety concerns were observed during the visit.

The facility underwent an unannounced Case Management visit to check on two newly admitted residents. The Licensing Program Analyst observed that both residents were in good condition, their rooms were sanitary, and accommodations were comfortable. Both residents reported that their needs are currently being met, and no deficiencies were cited in the report.
