Key Review Excerpts

The staff was especially kind in a time of need.

Long-term resident's family · 2024★★★★★

My 81 year old mom was here for rehab and was treated with such kindness. The staff was great! My dad is also handicap and they brought in a recliner in her room so that when he was there he could be comfortable, and when he came they offered him meals so that he could eat with my mom.

Rehab patient's family · 2024★★★★★

I was more than pleasantly surprised when I arrived. It smelled so nice and was very clean and cozy. The staff was so caring across all of the departments!

15deficiencies
Well above state avg (7.6)
18 complaint-triggered

Families have filed multiple complaints leading to serious deficiencies, including recent issues with abuse protection and reporting failures that persist across surveys. The facility repeatedly struggles with resident safety and accident prevention, abuse and neglect protections, and fire safety systems. While the provider reports corrections for each violation, the recurring pattern of complaint-triggered deficiencies in critical care areas suggests ongoing systemic problems that warrant careful consideration before placement.
