Key Review Excerpts

In about 6 months the staff at Trinity Fairborn has taken my father from a confused, angry, desperately ill diabetic with sugar levels through the roof, to a stable, healthy, grateful socialite who delights both staff and patient/clients.

Memory care family member · 2026★★★★★

My father in law was in there for 6 weeks & almost died due to neglect. He was severely dehydrated & his kidneys were shutting down. He hadn’t been shaved in 6 weeks & lost 32 lbs.

Long-term resident's family · 2022☆☆☆☆

The first thing you will notice. The place smells straight of death. I was suspicious that some of their residents was dead already. The staff started out nice. I was suspicious of them when my adopted mom went there and then later on found out when I left. She was left in a wet depend for over 4 hours before I came.

8 complaint-triggered

This facility shows concerning patterns with repeated issues across fire safety, resident rights, and medication management, plus families have filed eight complaints including serious matters involving neglect reporting and resident protection. The facility has struggled with the same problems across multiple years - fire safety deficiencies appeared in 2019, 2022, and 2025, while issues with notifying families of resident changes and medication services have also recurred. Though all violations show correction dates, the persistence of similar problems suggests ongoing compliance challenges that warrant careful consideration.
